Getting Started

Services start with a complimentary in-home or phone consultation with our Community Outreach Manager.

All it takes to start the process is one phone call. Our Community Outreach Manager will assist you in determining what type of service is needed for you or your loved one. The Community Outreach Manager will listen to your concerns and questions and will assist you in making an informed decision. If you decide that you would like to start services, the Community Outreach Manager will have you sign the Client Service Agreement and will collect a deposit for the services. Then we will schedule a nurse or the appropriate clinician to conduct the initial assessment.

Initial Assessment for Home Care Services

One of our RN Care Managers will conduct a complimentary Home Care Assessment and develop a personalized plan of care. Each plan of care addresses your needs and goals and instructs our care staff on how best to care for you. The assessment takes approximately one to one-and-a-half hours to complete. We will ask questions about the following:

  • Your medical history
  • Medications and how you manage them
  • Personal care such as dressing, grooming, and hygiene
  • Mobility needs such as assistive devices and recent falls
  • Toileting/incontinence assistance
  • Meal preparation
  • Housekeeping needs
  • Life enrichment

We generally start providing care within 24 to 72 hours after the initial assessment but we may be able to start sooner if there is an urgent need.

Initial Assessment for Home Health Services

Depending on the services required, a registered nurse, physical therapist, or occupational therapist will meet with you and assess your specific needs.  Together you and your CHCS clinician will decide upon a weekly schedule for nursing visits and/or therapy.  

Cost of Services

The cost of services varies depending on the type of service that is needed, the amount of care or number of hours required, and the location where services will be provided. Your CHCS care team will work with you to develop a plan for services that best meets your needs and budget.
