Parkinson's Disease Program

The Northwest Chapter of the American Parkinson Disease Association (APDA Northwest) sponsors a “Caregiver’s Day Off Program” which provides 20 hours of care per calendar year so family caregivers can take some time off for themselves.  We are proud to be one of the select agencies that partners with the APDA and supports the Parkinson’s Disease “Caregiver Day Off Program”.


How do I Apply for the “Caregiver Day Off Program”?

Call CHCS and our Community Outreach Manager will assist you with completing the simple Parkinson’s Association application. The application requires a doctor’s confirmation of the Parkinson’s disease diagnosis. We are happy to assist in obtaining the doctor’s signature. 

The approval process may take 7 to 10 days from receipt of the application. Once the application has been approved, one of our nurses will conduct an assessment and develop a personalized plan of care for your loved one. Our nurses have years of experience in caring for individuals with Parkinson’s disease and have completed in-depth training and skills verification with the caregiver(s) who will be providing the hands-on care in your home.  

Many clients and family caregivers want to continue with home care services beyond the 20 hours provided by APDA Northwest. Please call the CHCServices office for more information on continuing home care services.
