Home Health

Home health is skilled medical care provided by a registered nurse, physical therapist, occupational therapist, speech language pathologist, or other skilled medical professional.  Home health is ordered by a medical provider (physician, physician assistant, nurse practitioner) and is often prescribed as part of the plan of care following a hospitalization, a change in a medical condition, or when seniors experience an overall decline in functioning.

Nursing services can provide the following:

  • Assessment of medical condition
  • Medication reconciliation and medication management
  • Administration of medications and injections
  • Home safety assessment
  • Development of a fall reduction plan

  • Bladder and bowel management
  • Wound care
  • Client, family, and caregiver education and training
  • Development of a comprehensive plan of care 
  • Supportive and educational resources
  • Coordination of care with providers including physicians and therapists

Physical Therapy services can provide the following:

  • Evaluation of ability to walk safely
  • Training and restoration of walking, balance, and mobility
  • Home safety assessment
  • Development of a fall reduction plan   
  • Recommendations for wheelchair, walkers, crutches, or other mobility equipment
  • Training on the proper use of equipment such as walkers, Hoyer lifts, sliding boards
  • Training in overall strength and endurance secondary to deconditioning post illness
  • Client, family, and caregiver education and training
  • Development of a home exercise program

Occupational Therapy services can provide the following:

  • Evaluation of ability to perform activities of daily living including showering, dressing, and toileting
  • Development of a plan of care to improve ability to perform activities of daily living
  • Recommendations for adaptive equipment and training in its use
  • Recommendations for assistive technology and training in its use
  • Home safety assessment
  • Development of a fall reduction plan   
  • Home and workplace evaluation and recommendations for modification of  the environment
  • Manual therapy to reduce pain and improve function
  • Assessment of cognitive skills and visual perception 
  • Sensory Integration
  • Client, family, and caregiver education and training
  • Supportive and educational resources
  • Community reintegration   

Speech Language Pathology (speech therapy) services can provide the following:

  • Evaluation and treatment of swallowing and feeding disorders
  • Evaluation and treatment of speech and language disorders
  • Recommendations for augmentative and alternative communication systems and training in its use
  • Assessment and treatment of cognitive disorders
  • Development of strategies and tools to compensate for cognitive decline
  • Screening for hearing disorders
  • Supportive and educational resources
  • Client, family, and caregiver education and training

Sources for home health funding include personal insurance, Washington State Labor and Industries, and private pay. Please call if you would like more information about the insurance that we accept.

CHCServices is a Non-Medicare agency and cannot take Medicare insurance. Please call if you need assistance in finding a Medicare Home Health agency.

Home health services can also be provided to individuals who wish to pay privately for services. Private pay home health is convenient and a great way to supplement other health care services and maintain independence and safety in the home.
