Injured Worker Rehabilitation

CHC Services specializes in expert skilled rehabilitation and home care services for individuals recovering from injuries.

Our services focus on a comprehensive program to maintain and increase independence, safety and the necessary support to return the client to appropriate employment.

We use a team approach to management consisting of all of the necessary rehabilitation professionals, including a physiatrist and home health aides. We complete a comprehensive assessment, develop and implement an individualized rehabilitation plan, measure the plan results, do the appropriate follow up, provide regular reporting and work in collaboration with the Claims Manager, Case Manager, Vocational Counselor and treating physician.

Our Clients

Clients include persons injured on the job who may have the following attributes:

Would benefit from ongoing active medical management, such as wound care, to prevent medical complications, rehospitalizations and frequent ED visits
Requires an interdisciplinary approach to recovery and return to work
Has cognitive, behavioral, emotional, mobility or other impairments for which adaptations, equipment or other compensatory strategies may be needed for a successful reintegration into the community or workplace
Needs assistance with activities of daily living to promote functional independence and community adaptation
At-risk for developing chronic medical conditions secondary to the work-related injury
