Initial Admissions Assessment

The Initial Admissions Assessment is typically performed by the Care Manager. It usually occurs either in the Hospital or Facility setting, or the Home setting. The steps taken in these settings are as follows:

Hospital Setting - the Care Manager:

  1. Reviews medical records and interviews client, family, external case manager
  2. Administers cognitive assessment - if appropriate
  3. Administers balance and gait test - if appropriate
  4. Assesses for continence
  5. Assesses for safety awareness
  6. Compiles above information into patient profile document (pertinent personal information, history, physical, contact)
  7. Coordinates development of program team

Home Setting - the Care Manager:

  1. Reviews past medical history, medications, and interviews client, family, external case manager
  2. Contacts involved home health clinicians or outpatient clinicians for treatment plan
  3. Administers cognitive assessment - if appropriate
  4. Administers balance and gait test - if appropriate
  5. Assesses for continence
  6. Assesses for safety awareness - by having client demonstrate safety skills
  7. Compiles above information into patient profile document (pertinent personal information, history, physical, contact)
  8. Coordinates development of program team
