Home Care Services

Home Care Services provide one-on-one care with certified nursing assistants. It is designed to help an individual remain in their home while meeting their everyday needs in a safe and effective manner.

All of our services begin with a nursing assessment and a plan of care is created, with the family's approval, to address the needs of our client.

Home Care Services are provided by our highly trained Care Specialists. All are certified or registered nursing assistants and supervised by Registered or Licensed Nurses.

CHC Services offers Nurse Delegation, which allows Care Specialists to perform certain tasks that require oversight and training by an RN, but may be performed more economically by a Care Specialist. Examples of these tasks are insulin administration, dressing changes and tube feedings.

Care Specialists

Our Care Specialists go through a rigorous hiring process and extensive national background check and orientation. Care Specialists are certified or registered nursing assistants who are primarily responsible for the day-to-day implementation of the Care Plan with the client. They provide the one-on-one care, coaching, and assistance in the activities around the home and in the community to clients.

CHC Services values and has a very strong emphasis on ongoing training and education of the Care Specialist staff. The training includes them as a part of the team with the Medical Director, Care Manager, and licensed clinicians. The Care Specialists activities are driven by the details of the individual's Care Plan and may include:

Personal care
Medication reminders
Safety checks
Meal preparation
Recreational activities
Exercise programs
Light housecleaning
Transportation to and from appointments

Our home care clients include those who:

May be at risk for being unsafe alone at home
Would like to remain in their home (house, retirement community, assisted living facility or adult family home)
Are suffering from dementia or Alzheimer's disease
Have loved ones who live far away
Have significantly compromised health status
Have a new or complex diagnosis
Are significantly impaired in their mobility
Are impaired cognitively or behaviorally and need support and supervision
Are recovering after discharge from a hospital, nursing home, or rehabilitation facility
