CHCServices is a provider of comprehensive home health and in-home care and companion services

We are a local, doctor owned, accredited, and licensed home healthcare company whose goal is to help individuals remain in their homes or community by providing skilled healthcare services such as nursing or physical or occupational therapy, companion or certified aide services, and care management or disease management services.

We offer primarily three services to assist in meeting our clients’ needs: Care Management Services, Companion Home Care Services, and Skilled Home Health Care Services. However, Care Management is central to all that we do at CHC Services.

Care Management Services

Care Managers are experienced nurses and clinicians that help individuals and families manage the complexities of a new or chronic illness or disability. Clients can receive Care Management as an integral component of a comprehensive set of services or as a solitary service.

Home Health Care Services

Home Health Care Services or Skilled Healthcare Services such as therapy services including physical, occupational, speech, recreational, nutrition counseling, and more are also available through CHC Services. The therapy can be provided individually or together as part of a comprehensive rehabilitation plan. Additionally we also provide drivers evaluation and rehabilitation, evaluation for home modifications, assistive technology evaluations, and wheelchair prescriptions.

Home Care Services, Companion and Aide Services

Companion or aide services are non-medical services designed to help an individual meet their everyday needs in a safe and effective manner. Such services are provided by our compassionate, qualified, and responsible Care Specialists who undergo extensive background checks and screening prior to employment. Care Specialist services are always provided in the context of a well-defined Care Plan with medical/nursing oversight.

Care Specialist services include assistance with all personal care, bathing, cooking, housekeeping and companionship. In addition with specialized training, delegation, and supervision by an RN, an aide can perform more complicated tasks such as blood glucose monitoring, non-sterile dressing changes, insulin administration, and more.

Other Services

We offer a variety of other services including telehealth and biometic services and equipment to support independence and safety in the individual’s living environment. The MD2 personal medication system is one such product that promotes independence and safety with medication management. It has built in reminding, monitoring, and will notify up to four family members, friends, and/or care managers by telephone if medications are missed. Call for more information 425-275-5858.
